Who Needs a DOT Physical?

DOT physicals are required for:

  • CDL applicants and current CDL holders

  • Interstate commercial vehicle drivers

  • Drivers operating vehicles over 10,000 lbs

  • Drivers transporting hazardous materials

If you’re unsure whether you need an exam, we’re happy to help clarify requirements.

What’s Included in Your DOT Exam:

Your DOT physical includes all federally mandated components:

  • Basic vitals 

  • Vision testing 

  • Hearing test 

  • Urinalysis 

  • Review of medical history and current medications

  • Physical examination based on FMCSA guidelines

If you meet the requirements, you’ll receive your Medical Examiner’s Certificate (DOT card) on-site.

How to prepare for your appointment:

To make your appointment as smooth and fast as possible, please bring:

  • Your current driver’s license

  • Eyeglasses or contacts (if you use them)

  • Hearing aids (if applicable)

  • List of current medications

  • Recent medical records related to chronic conditions, surgeries, sleep apnea, diabetes, cardiac conditions, or any other items that may require documentation

  • DOT Physical Exam: $100

  • Deposit Required: $50 (to secure your appointment)

DOT Health Requirements Overview

To pass your DOT physical, you must meet FMCSA minimum standards, including:

​

  • Vision: 20/40 correctable vision in each eye and both eyes together

  • Color Vision: Must be able to distinguish basic colors

  • Hearing: Ability to perceive a forced whisper at 5 feet

  • Blood Sugar: Diabetes without insulin injections is acceptable; controlled diabetes permitted

  • Substance Use: No use of Schedule I substances, amphetamines, narcotics, or habit-forming drugs
